Provides individual, family and group clinical services to patients in

the Methadone Program in order to treat substance abuse issues, assess

and refer for mental health and other appropriate services, and

improve life functioning. Works as part of an interdisciplinary team

to assess, develop and implement treatment plans for recovery.


Responsible for maintaining an active caseload of assigned patients

providing individual, family and group treatment along with case

management. Frequency of sessions based on clinical need and assigned

levels of care.

Completes psychosocial assessment, quarterly treatment plans, periodic

assessments, progress notes, discharge summaries and other related

documentation as required within established timeframes.

Provides crisis assessments and management in consultation with


Responsible for tracking patient compliance with program requirements

and issues warnings and contracts as clinically indicated. Attends

appeal hearings.

Completes all documentation within required time frames and submits

each day s encounter forms for billing daily.

Practices according to relevant DPH, Joint Commission, SPHS and MMTP

regulations and standards.

Maintains professional relationships and boundaries with patients,

co-workers and the community.

Fully competent bilingual employees will be responsible for providing

Maintains licensure and credentials through the attainment of

Work requires the knowledge of theories, principles, and concepts

normally acquired through the completion of a Master s Degree in

Social Work or clinically focused Human Service field.

Work requires a solid clinical knowledge base of theoretical

approaches and interventions required to meet the needs of a diverse

client base.

Licensed or license-eligible preferred.

Requires two (2) years related experience in mental health and

substance abuse settings with a minimum of one (1) year of substance

abuse experience. Post Master's experience preferred.

Bilingual staff will require successful completion of a language

competency exam prior to beginning employment.

Requires good computer skills with the ability to document in the

electronic medical record.

Requires good organization, time management, communication, and

documentation skills.
